I made this for gym class heroes But don’t have any contacts someone needs to hook it up. when i transferred it from illustrator to photo shop it lost resolution if any one can help i will love you for ever and thanks jimiyo for the template

you should be able to copy and paste from illustrator to photoshop fine. It usually gives you some options, choose "smart object" to maintain resolution.
